Nurse (Clinical/Case Management)

About the Position: This position is with the Defense Health Agency at the Carl R. Darnall Army Medical Center in Fort Hood, Texas This is a Direct Hire Solicitation. A recruitment or relocation incentive may be authorized. Student Loan Repayment may be authorized. Salary negotiation may be available for those candidates who are new to Federal service.

Provide care coordination for patients with complex medical conditions; assess patient needs, create care plans, monitor progress, and adjust plans as needed. Support patients and families through education; teach patients and caregivers about their conditions, warning signs, medications, and self-management. Coordinate care across the healthcare team; work with doctors, nurses, and other providers to ensure smooth transitions and consistent care. Identify patients who need case management; review patient records and data to find individuals with chronic conditions, frequent hospital visits, or other high-risk factors. Lead and mentor clinic staff; provide guidance, training, and support to nursing staff and assist with quality improvement and accreditation activities.
