Nurse (Educator/Diabetes)

About the Position: This position is located in the Department of Medicine, Endocrinology Service at Brooke Army Medical Center at Fort Sam Houston in San Antonio, Texas. This is a direct hire solicitation. A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes.

Provide education to diabetes patients and their families in accordance with the National Standards for Diabetes Self-Management Education. Develop, implement ,and evaluate individual adult diabetes self-management education plans for active duty soldiers, their dependents and civilian adult beneficiaries. Collaborate with other members of the clinical team to ensure that patients are receiving appropriate diabetes care. Design classes for each target audience. Ensure that necessary supplies are available for the classes. Review diabetes education program consults, prioritize high-risk patients and schedules patients for the education classes. Provide initial and follow up diabetes self-management care for diabetes patients.
