Job description
The Advanced Practice Nurse (APN) provides advanced clinical practice, consultation, and management. The Advanced Practice Nurse(APN) practices to the full extent of their education, training, and certification. Emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission.
The Geriatrics and Extended Care Advanced Nurse Practitioner (ANP) Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork. Participates in interdisciplinary teams and processes that positively impact patient outcomes. Make visits to patient homes and assisted living facilities to deliver primary care services. Duties include but not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrate leadership and work collaboratively with the interdisciplinary team. Order and interprets labs, tests, and diagnostic studies. Prescribe pharmacologic and non-pharmacologic interventions. Initiate referrals and consults to specialty services. Provide education to patients and their families. Assesse and adjusts the plan of care as needed to achieve optimal patient outcomes. Independently practice as a NP providing a full range of services utilizing treatment guidelines in the delivery of high-quality care. Collaborate with patients, families, and health care practitioners to develop, implement, and evaluate care. Utilize customer service principles, supports veteran self-determination, and serves as a resource for patients and staff in addressing ethical issues. Demonstrate critical thinking in assessing and managing complex situations. . Lead the delivery of care to assure continuity of care and peer accountability for practice, improving outcomes of panel/program. Engage in evidence-based practice and the application of clinical practice guidelines. Implements quality improvement initiatives that lead to outcomes at the panel/program level. Demonstrate positive effective communication skills and professionalism that promote cooperation and teamwork with internal and external customers. Work effectively as a leader in planning and facilitating systematic change to improve outcomes. Other duties as assigned.
