Job description

The Registered Nurse (RN) Primary Care Patient Aligned Care Team (PACT) Coordinator will demonstrate proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations. Serves in an administrative role as a PACT champion and coordinates local PACT implementation, training, communication, and sustainability of the PACT model. Actively promotes the PACT model within the Ambulatory Care setting, educates core PACT team members.

Duties are but not limited to: The RN identifies and addresses communication and workflow challenges, collaborates with leadership and associated stakeholders, and schedules meetings among teams. Follows procedures per established policies and guidelines. Influences outcomes by collaborating with members of the interdisciplinary team. The Nurse PACT Coordinator is responsible for providing competent, evidence-based practices. Validates understanding of the rationale for PACT care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. Demonstrates the ability to cope with and manage competing priorities. Integrates knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations. The nurse demonstrates competence in practice and decision-making, deliberate planning, and critical thinking skills. Executes position to promote full PACT implementation and function within the Primary Care setting. Focuses on the implementation and sustainment of the PACT model driven by quality improvement and evidence-based decision-making. Interacts with and assists PACT members in achieving compliance with Veterans Affairs Central Office (VACO), Veteran Integrated Services Network (VISN), and local policies and directives. Assists teams to maintain national Primary Care standards using clinical practice guidelines and clinical pathways. Promotes PACT fundamentals. Conducts knowledge sharing to provide safe, efficient, quality PACT care. Fosters a safe and supportive environment of care. Delivers self-directed, professional nursing practice using the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) for complex patients based on conscious and deliberate planning with occasional supervision. Expectations: 1. Uses knowledge of PACT principles to support PACT team care delivery, resulting in positive impact/improvement. Outcomes clearly demonstrated. 2. Assists core PACT teams in organizing care delivery. 3. Monitors fidelity to the PACT model of care. Monitors outcomes. Offers focused training and education relating to the PACT model of care. 4. Independently plans and delivers technically competent care to patients with complex needs in alignment with the ANA Standards of Nursing Practice. 5. Manages the care of patients, while organizing and prioritizing patient care needs, and ensuring assigned tasks are completed. 6. Functions as an emerging leader in providing direction to staff to identify, implement and evaluate patient care needs. 7. Collaborates with the interdisciplinary team to ensure the plan of care is achieved. Preferred Experience: 3-5 years PACT experience, Chronic Disease Management, ability to create excel data spreadsheets, Charge Nurse experience, ability to analyze data, educate staff on performance measure to meet eQM and SAIL metrics by utilizing multiple databases, develop and complete data reports utilizing excel and PowerPoint presentations.
